People who have certain intellectual/developmental disabilities need help getting the care they need. TennCare members who get waiver services from the Department of Intellectual and Developmental Disabilities (DIDD) may be eligible for SelectCommunity.

The SelectCommunity program helps people with intellectual/developmental disabilities.

Personal Help from a Nurse Care Manager

SelectCommunity offers members integrated care management. Each member is assigned a Nurse Care Manager. He or she creates an individualized plan to make sure each member gets the services he/she needs. Then the Nurse Care Manager works with all the people involved in the member's care to help get them that care.*

The Nurse Care Manager works with:

  • each member
  • the people who take care of the member, like DIDD, residential providers, doctors and family
  • the member's Independent Support Coordinator (ISC) or Waiver Case Manager
Who Can be Part of SelectCommunity?

SelectCommunity is for people who have intellectual/developmental disabilities. A person may be eligible for SelectCommunity if:

  • they are a TennCare member AND
  • they already get services through the State of Tennessee's Home and Community Based Services (HCBS) waiver programs for people with intellectual/developmental disabilities

* Services, programs or therapies that may be needed are not covered by all health plans. Health plans also may have limits on the care and services members receive. If members go over their plan's limits, they may have to pay for care or services themselves. Always check your plan to learn what is covered or what you may have to pay for yourself. If you have questions, call the Customer Service number listed on your member ID card.
