Our members tell us they like help when they face health challenges, and they want support to stay healthy. That’s why we offer our CareSmart® Population Health Program.
You may only need information and reminders about healthy habits. You may need more in-depth attention and be assigned to a case manager for individual health coaching (see below).
Our caring staff is here for you whether you or your child are well, have an ongoing health problem or a serious health episode. It’s part of your membership, so there’s no cost. It’s voluntary, so you can leave the program at any time.
