Long Term Services and Supports

The Long Term Services and Supports (LTSS) program includes a wide range of benefits for members. Providers in the BlueCare Tennessee network serve these members through three separate programs.

CHOICES

CHOICES providers serve members in nursing facilities, as well as at home or in the community, known as Home and Community Based Services (HCBS). Find more information about the program, including manuals, training and forms.

Employment and Community First (ECF) CHOICES

SelectCommunity

SelectCommunity is a TennCareSelect program open primarily to people with intellectual developmental disabilities who are enrolled in one of the State’s Section 1915(c) Home and Community Based Services Waiver programs, as well as former Arlington class waiver persons residing in a private Intermediate Care Facility for persons with Intellectual/Developmental Disabilities (ICF/ID).

Members are assigned a Nurse Care Manager (NCM) who serves as the member's and the provider's primary point of contact for physical and behavioral health needs.

1915(c) Waiver Program

The 1915(c) waivers offer services to people with intellectual disabilities to help them live, work and be involved in their communities, with as much self-determination and independence as possible. The services are called “Home and Community Based Services” or “HCBS” and are an alternative to institutional care. The three 1915(c) waivers are the Self-Determination (SD) Waiver, Statewide (SW) Waiver, and Comprehensive Aggregate Cap (CAC) Waiver. These waivers are closed to new enrollment as of July 1, 2016.

ICF/IID

An ICF/IID provides specialized services for individuals with an intellectual disability or related conditions. These facilities are required to provide active treatment through intensive specialized supports and services designed to assist individuals with intellectual disabilities to develop increased skills and independence in functional life areas which are preventing the individual from living in a more independent setting.
