CHOICES providers serve members in nursing facilities, as well as at home or in the community, known as Home and Community Based Services (HCBS).

How to apply as CHOICES Provider

Thank you for your interest in participating as a CHOICES provider in the BlueCare Tennessee network. To begin the application process, please call the BlueCare Provider Service Line 1-800-468-9736. The following items are all required before you can participate as a network provider:
