ECF CHOICES providers promote and support integrated, competitive employment and independent living as the first and preferred option for members with intellectual and developmental disabilities (I/DD). ECF is a different way to think, plan and support people with I/DD. We’re looking for providers who have the skills and compassion to meet the needs of this unique population.

ECF Provider Application Requirements
- Applicable licensure
- W-9
- Form 147 IRS
- Proof of DIDD licensure
- Liability Insurance (Professional and General) (Automobile): Minimum Coverage $500,000)
- Disclosure of Ownership and Control Interest Statement: Form must be completed in its entirety, marking non-applicable items with an N/A, signed and dated.
