Collect these documents before submitting your application:
Applicable licensure
W-9
Form 147 IRS
Proof of Department of Disability and Aging (DDA) licensure (formerly known as DIDD)
Liability insurance (professional, general and/or automobile – minimum coverage $500,000)
Disclosure of Ownership and Control Interest Statement: Form must be completed in its entirety, marking non-applicable items with “N/A,” signed and dated

Helping members work independently

Employment allows people an opportunity to be a part of their community, gain earned income, enhance skills and knowledge, and increase their self-esteem and independence, regardless of disability status. Every person's employment journey is specific to them and the services they receive are no different. The employment services offered through ECF CHOICES ensure each person receives the type of supports they need to be successful. Services are individualized, person-centered, and allow the person the autonomy to decide how their employment journey will look.
