Personal Representatives
I'm Representing a Member
A personal representative (PR) can legally make health care decisions for a member. You don't need to be a member to be a PR. You might be a parent or legal guardian. If you would like to have online access to a member's account as their PR, we need to check your PR status first. Then you can set up an account and link it to theirs.
Here are a few things you need to have ready when you ask for a PR account:
- The Member ID card for the member you represent
- Documents that show you're allowed to make health care decisions for this member
These documents could be:
- Birth or death certificate
- Legal documents that prove you represent a member and can make health care decisions for them
- Court orders
- Other supporting documents
